About The Position

WellBe Senior Medical is hiring a Nurse Practitioner to deliver value-based primary care directly in patients’ homes in Hermitage, Pennsylvania. The role focuses on whole-person care, patient autonomy, and outcomes rather than volume. The Nurse Practitioner manages a dedicated patient panel with support from an interdisciplinary care team, including daily team check-ins, ongoing training, and one-on-one mentorship. The position includes a $10,000 sign-on bonus, a salary of $104,763–$166,750, mileage, full benefits, and relocation availability. The role requires local field-based travel and employment is contingent on pre-employment drug screening and a background check, with monthly FACIS checks during employment. WellBe does not provide employment-based visa sponsorship.

Requirements

Master’s degree as a Nurse Practitioner.
Active, unencumbered Nurse Practitioner license in Pennsylvania.
Board certification through ANCC or AANP.
Valid driver’s license and a vehicle for local field-based travel.
Legally authorized to work in the United States without employer sponsorship now or in the future.
Successful completion of a pre-employment drug screening and background check; continued employment is contingent on satisfactory monthly FACIS checks.

Responsibilities

Conduct comprehensive primary care visits and physical exams in patients’ homes.
Create longitudinal care plans to manage chronic disease in high-risk patients.
Partner with an interdisciplinary team to support seamless, safe care transitions.
Treat acute needs in the home to help prevent emergency room visits and hospitalizations.
Address social determinants of health as part of whole-person care.
Manage a dedicated patient panel with support from the interdisciplinary care team.
