About The Position

WellBe Senior Medical is hiring a Nurse Practitioner to deliver value-based primary care directly in patients’ homes in the Harrisburg, Pennsylvania area. The role focuses on whole-person care, including social determinants of health, and provides autonomy, a supported patient panel, and collaboration with an interdisciplinary care team. The position includes a $10,000 sign-on bonus, mileage, full benefits, and relocation availability. The Nurse Practitioner conducts comprehensive primary care visits and physical exams in patients’ homes, develops longitudinal care plans for high-risk patients, collaborates with an interdisciplinary team to support safe care transitions, and treats acute needs at home to help prevent emergency room visits and hospitalization. WellBe offers daily team check-ins, ongoing training, and one-to-one mentorship.

Requirements

Master’s degree as a Nurse Practitioner.
Active, unencumbered Nurse Practitioner license in Pennsylvania.
Board certification through ANCC or AANP.
Valid driver’s license and a vehicle for local field-based travel.
Must be legally authorized to work in the United States without current or future employer visa sponsorship.
Must successfully complete a pre-employment drug screening and satisfy applicable ongoing drug-free workplace testing requirements.
Employment is contingent upon successful completion of a background check; employees are subject to monthly FACIS checks.

Nice To Haves
