About The Position

WellBe Senior Medical is seeking a Nurse Practitioner to deliver value-based primary care to seniors in their homes in Waynesburg, Pennsylvania. The role emphasizes whole-person, outcomes-based care, with autonomy and support from an interdisciplinary team. The Nurse Practitioner conducts comprehensive primary care visits and physical exams, manages chronic conditions through longitudinal care plans, coordinates care transitions, and treats acute needs at home to help prevent emergency room visits and hospitalizations. The position includes a dedicated patient panel, daily team check-ins, ongoing training, and one-to-one mentorship. Salary is $104,763–$166,750, plus mileage and full benefits. A $10,000 sign-on bonus is offered, and relocation is available.

Requirements

Master’s degree as a Nurse Practitioner.
Active, unencumbered Nurse Practitioner license in Pennsylvania.
Board certification through ANCC or AANP.
Valid driver’s license and a vehicle for local field-based travel.
Must be legally authorized to work in the United States without current or future employer visa sponsorship.
Must successfully complete a pre-employment drug screening and satisfy applicable ongoing drug-testing and background-check requirements, including monthly FACIS checks.

Nice To Haves

Master’s degree with a preferred focus in Adult, Geriatric, Acute Care, and/or Family practice.
