About The Position

Senior LIFE is an innovative home- and community-based Medicare and Medicaid-funded program that provides all-inclusive healthcare services and support to seniors living in the community. The Registered Nurse helps seniors remain at home by identifying and addressing their healthcare needs and partnering with members and Senior LIFE interdisciplinary teams to provide care. The position is full-time in Waynesburg, Pennsylvania, with a Monday–Friday daylight schedule from 8:00 AM to 4:30 PM and a minimal on-call rotation.

Requirements

• At least one year of experience working specifically with an elderly or aging population.
• Current RN license.
• Valid driver's license.

Responsibilities

• Identify medical needs for individual members and set care priorities accordingly.
• Develop ongoing Plans of Care in partnership with Senior LIFE interdisciplinary teams.
• Interpret, record, and act on diagnostic results; perform advanced nursing techniques and critical care procedures.
• Identify and assess major medical problems and communicate potential needs to a physician or specialist.
