About The Position
Senior LIFE is an innovative home- and community-based Medicare and Medicaid funded program providing all-inclusive healthcare services and support to seniors living in the community. The program helps seniors remain at home in familiar surroundings while receiving the care they need. Staff partner with seniors to promote open communication and shared decision-making while providing excellent care and services. The Registered Nurse identifies members’ medical needs, develops ongoing plans of care with interdisciplinary teams, interprets and acts on diagnostic results, performs advanced nursing techniques and critical care procedures, and assesses significant medical problems while communicating potential needs to physicians or specialists.
Requirements
- At least one year of experience working specifically with an elderly or aging population.
- Current RN license.
- Valid driver's license.
Responsibilities
- Identify medical needs for individual members and set care priorities accordingly.
- Develop ongoing plans of care in partnership with Senior LIFE interdisciplinary teams.
- Interpret, record, and act on diagnostic results.
- Perform advanced nursing techniques and critical care procedures.
- Identify and assess major medical problems and communicate potential needs to a physician or specialist.
