About The Position

WellBe Senior Medical is seeking a Nurse Practitioner to provide value-based care directly in patients’ homes in Hermitage, Pennsylvania. The role focuses on whole-person primary care for seniors, with autonomy, support from an interdisciplinary care team, daily team check-ins, ongoing training, and one-to-one mentorship. The position offers a $10,000 sign-on bonus, a salary of $104,763–$166,750, mileage, full benefits, and relocation assistance. The Nurse Practitioner conducts comprehensive primary care visits and physical exams in patients’ homes, develops longitudinal care plans for high-risk patients, collaborates with an interdisciplinary team to support safe care transitions, and treats acute needs at home to help prevent emergency room visits and hospitalizations.

Requirements

Master’s degree as a Nurse Practitioner.
Active, unencumbered Nurse Practitioner license in Pennsylvania.
Board certification through ANCC or AANP.
Valid driver’s license and a vehicle for local field-based travel.
Must be legally authorized to work in the United States without current or future employer-based visa sponsorship.
Must successfully complete a pre-employment drug screening and background check; continued employment is contingent on applicable drug-testing and FACIS check requirements.
