About The Position

Matrix Medical Network is hiring a PRN Nurse Practitioner to conduct comprehensive in-home health assessments for members, including older adults and other at-risk individuals. During visits lasting up to one hour, providers review members’ current health, medical history, medication adherence, social environment, and other risks. Assessments can help identify chronic conditions and close care gaps that may not be addressed during routine office visits.

Providers deliver services in members’ residences, including private homes and Skilled Nursing Facilities. Visits are scheduled according to provider availability and may take place any day from Sunday through Saturday, between 8 a.m. and 7 p.m. Compensation is based on completed assessments, with the posting stating that providers averaging 20–100 visits monthly earn $1,800–$11,500 per month, at a per-assessment rate of $90–$115 depending on state of residence.

Matrix provides onboarding, a preceptor, individual feedback, and free access to continuing education. Where business needs exist, the company may support additional state licensure and credentialing. PRN providers may transition to full-time positions and receive a full-time sign-on bonus.

Requirements

• Master’s degree or commensurate experience and satisfactory completion of Nurse Practitioner licensure.
• Current NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority as needed.
• Board certification from AANP, AACN, or ANCC in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS, or CPR certification.
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage required for the state of residence, or the ability to obtain it, with bodily injury coverage of $100,000 per person/$300,000 per incident, property damage coverage of $50,000, and collision and comprehensive deductibles of $1,000 or less.
• Strong computer skills and familiarity with employee health or medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult and geriatric assessments covering medical history, diagnosis and treatment, health education, physician referrals, case management referrals, and follow-up.
• Document assessments clearly according to Matrix guidelines and protocols.
• Administer and order point-of-care and lab-based testing as needed.
• Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Collaborate with Primary Care Physicians on patient education and follow-up as necessary.
• Provide services at members’ places of residence, including homes and Skilled Nursing Facilities.
