About The Position

Matrix Medical Network is hiring a PRN Nurse Practitioner to conduct in-home health assessments for adults and older adults, including members who may be at risk or have chronic conditions that have not been diagnosed. Visits can last up to one hour and take place in members’ homes or Skilled Nursing Facilities. Providers review and observe members’ current health, medical history, medication adherence, social environment, and other risks, helping identify care gaps and improve quality of care. The role offers flexible scheduling based on provider availability, with assessments scheduled Sunday through Saturday from 8 a.m. to 7 p.m. Compensation is based on completed assessments, at a stated rate of $90–$115 per assessment depending on the state of residence. Providers completing an average of 20–100 visits per month may earn $1,800–$11,500 per month. PRN providers may have the opportunity to transition to full-time roles; a full-time sign-on bonus is mentioned for those opportunities. Matrix provides onboarding, a preceptor, one-to-one feedback, access to continuing education, and may support additional state licensure and credentialing when business needs exist. The position includes mileage reimbursement, a 401(k), employee referral bonuses, and limited voluntary benefits.

Requirements

Master’s degree or commensurate experience and satisfactory completion of Nurse Practitioner licensure.
Current Nurse Practitioner licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority as needed.
Board certification from AANP, AACN, or ANCC in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
Current BLS, ACLS, or CPR certification.
Valid state driver’s license for the state of residence.
Automobile insurance coverage required for the state of residence, or ability to obtain it: bodily injury coverage of $100,000 per person/$300,000 per incident; property damage coverage of $50,000; and collision and comprehensive deductibles of $1,000 or less.
Strong computer skills and familiarity with employee health/medical record software.
Excellent verbal and written communication skills.

Responsibilities

Conduct adult and geriatric assessments, including medical history review, diagnosis and treatment, health education, physician referrals, case-management referrals, follow-up, and documentation according to Matrix guidelines and protocols.
Administer and order point-of-care and lab-based testing as needed.
Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
Collaborate with the Primary Care Physician on patient education and follow-up as necessary.
Provide services at members’ residences, including homes and Skilled Nursing Facilities.
