About The Position

Matrix Medical Network is seeking a Nurse Practitioner to conduct in-home Comprehensive Health Assessments for adult and geriatric members, including members living in Skilled Nursing Facilities. Visits may last up to one hour and are scheduled according to the provider’s availability. During each visit, providers review and observe the member’s current health, medical history, medication adherence, social environment, and other risks. The assessments are intended to improve quality of care and help identify chronic conditions and close care gaps that may not be captured during routine office visits. This PRN role offers compensation based on the number of completed assessments, with scheduling flexibility across available assessment hours. Matrix provides onboarding, a preceptor, individual feedback, and access to continuing education. Providers may be supported in obtaining additional state licensure and credentialing when business needs exist. PRN providers may also have the opportunity to transition to full-time roles.
