About The Position

Matrix Medical Network is seeking a Nurse Practitioner to conduct in-home Comprehensive Health Assessments for adult and geriatric members. Visits can last up to one hour and involve reviewing and observing members’ current health, medical history, medication adherence, social environment, and other risks. Assessments help improve quality of care and may identify chronic conditions or close multiple care gaps that routine office visits may not capture. Providers deliver services at members’ residences, including private homes and Skilled Nursing Facilities. Visits are scheduled around provider availability and may take place during the day, evening, or weekend. The role is PRN, with compensation based on completed assessments. The posting also notes that PRN providers may transition to full-time opportunities.
