About The Position

Matrix Medical Network is seeking a PRN Nurse Practitioner to conduct in-home and facility-based Comprehensive Health Assessments for adults and older adults. Visits can last up to one hour and involve reviewing and observing a member’s health, medical history, medication adherence, social environment, and other risks. The assessments help improve quality of care and may identify chronic conditions and close multiple care gaps. Providers see members at their residences, including private homes and Skilled Nursing Facilities. Visits are scheduled according to provider availability, with assessments offered Sunday through Saturday from 8 a.m. to 7 p.m. Compensation is based on completed assessments, with rates ranging from $90 to $115 per assessment depending on state of residence. PRN providers may have opportunities to transition to full-time roles.

Requirements

Master’s degree or commensurate experience and satisfactory completion of NP licensure.
Current NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority as needed.
Board certification from AANP, AACN, or ANCC in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
Current BLS, ACLS, or CPR certification.
Valid state driver’s license for the state of residence.
Automobile insurance coverage required for the state of residence, or ability to obtain it: bodily injury coverage of $100,000 per person/$300,000 per incident; property damage coverage of $50,000; and collision and comprehensive deductibles of $1,000 or less.
Strong computer skills and familiarity with employee health or medical record software.
Excellent verbal and written communication skills.

Responsibilities

Conduct adult and geriatric assessments, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, and follow-up.
Document assessments clearly according to Matrix guidelines and protocols.
Administer and order point-of-care and laboratory-based testing as needed.
Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
Collaborate with the Primary Care Physician on patient education and follow-up as necessary.
Provide services at members’ residences, including homes and Skilled Nursing Facilities.
