About The Position

Total Life is seeking nurse practitioners in the Tampa Bay–Lakeland area to lead longevity care days at partner senior living communities. The role combines a brief educational presentation for residents about healthy aging with one-on-one longevity visits for residents who sign up. The program is designed for older adults and combines comprehensive geriatric assessment with chronic disease management, cognitive health, medication review, and weight management. It complements rather than replaces residents’ primary care. The nurse practitioner will receive training and support from Total Life’s Chief Medical Officer, a board-certified geriatrician, and will be accompanied by a team member on the first site day. Site days take place on weekdays. The educational presentation is the fixed portion of each day, while the remaining schedule depends on resident sign-ups. Expect approximately 4 to 8 hours per site day, a few times per month initially, with hours increasing as bookings grow. This is an in-person contract role requiring travel to assigned senior living communities in the Tampa Bay–Lakeland area.

Requirements

  • Active, unrestricted Florida APRN license as a nurse practitioner from the Florida Board of Nursing, plus an active Florida or multistate RN license; a multistate RN compact license does not cover APRN practice.
  • Current national nurse practitioner board certification.
  • Active DEA registration and completion of Florida’s required 3-hour controlled substance prescribing course.
  • Ability to practice under a written protocol with a supervising physician as required in Florida, or registration as an Autonomous APRN with the Florida Board of Nursing, requiring 3,000+ supervised clinical hours in primary care.
  • Clinical experience caring for adults aged 65 and older.
  • Experience with Medicare documentation and E/M coding.
  • Own malpractice insurance with at least $1 million per occurrence and $3 million aggregate that covers in-person care, or ability to obtain such insurance before the first site day.
  • Comfort presenting to groups and speaking with older adults and their families.
  • Valid driver’s license and reliable transportation.
  • Ability to complete each community’s onboarding requirements, which may include a background check, TB screening, and immunizations.
  • Weekday availability for on-site days in the Tampa Bay–Lakeland area, with hours confirmed in advance.

Nice To Haves

  • Experience with comprehensive geriatric assessment.
  • Familiarity with cognitive screening and staging tools such as Mini-Cog, MoCA, and FAST; MoCA certification is a plus.
  • Experience in senior living, long-term care, skilled nursing facilities, hospice, palliative care, or house-call settings.
  • Experience prescribing GLP-1s for weight management.
  • Telehealth experience.
  • Licenses in more than one state.
  • Adult-Gerontology NP certification (AGNP/AGPCNP), or Family NP certification with a geriatric focus or training.

Responsibilities

  • Lead healthy-aging educational presentations for residents using materials provided by Total Life.
  • Conduct 45- to 60-minute longevity visits using a comprehensive geriatric assessment, including residents’ priorities, medication review and deprescribing, cognition, mood, sleep, nutrition, mobility, fall risk, daily function, and advance care planning.
  • Evaluate eligible residents for weight management, including GLP-1 therapy when clinically appropriate.
  • Identify residents who may benefit from additional care, such as a fuller cognitive review, ongoing medical follow-up, psychiatry, or therapy, and coordinate with their primary care provider.
  • Document every encounter accurately and completely in the EHR in accordance with Medicare standards.
  • Represent Total Life professionally with residents, families, and community staff.
  • Travel to assigned senior living communities in the Tampa Bay–Lakeland area.

Benefits
