About The Position

Total Life is seeking a nurse practitioner in the Tampa Bay–Lakeland area to lead longevity care days at partner senior living communities. Each site day begins with a short healthy-aging presentation using materials provided by Total Life, followed by one-on-one longevity visits with residents who sign up. The program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review, and weight management, and complements rather than replaces residents’ primary care. The nurse practitioner will receive training and support from Total Life’s Chief Medical Officer, a board-certified geriatrician. A Total Life team member will accompany the practitioner on the first site day. Site days take place on weekdays and generally last about 4 to 8 hours, depending on resident sign-ups. Work is expected a few times per month initially, with hours growing as bookings increase. Hours are confirmed in advance. The role requires travel to assigned senior living communities in the Tampa Bay–Lakeland area.

Requirements

  • Active, unrestricted Florida APRN license as a nurse practitioner from the Florida Board of Nursing, plus an active Florida or multistate RN license; a multistate RN compact license does not cover APRN practice.
  • Current national nurse practitioner board certification.
  • Active DEA registration and completion of Florida’s required 3-hour controlled substance prescribing course.
  • Ability to practice under a written protocol with a supervising physician, as required in Florida, or registration as an Autonomous APRN with the Florida Board of Nursing, requiring 3,000 or more supervised clinical hours in primary care.
  • Clinical experience caring for adults aged 65 and older.
  • Experience with Medicare documentation and E/M coding.
  • Own malpractice insurance of at least $1 million per occurrence and $3 million aggregate that covers in-person care, or ability to obtain it before the first site day.
  • Comfort presenting to groups and speaking with older adults and their families.
  • Valid driver’s license and reliable transportation.
  • Ability to complete each community’s onboarding requirements, which may include a background check, TB screening, and immunizations.
  • Weekday availability for on-site days in the Tampa Bay–Lakeland area, with hours confirmed in advance.

  • Familiarity with cognitive screening and staging tools such as Mini-Cog, MoCA, and FAST; MoCA certification is a plus.

  • Lead healthy-aging educational presentations for residents using provided presentation materials and speaker notes.
  • Conduct 45- to 60-minute longevity visits based on a comprehensive geriatric assessment, covering residents’ priorities, medication review and deprescribing, cognition including the Mini-Cog, mood, sleep, nutrition, mobility, fall risk, daily function, and advance care planning.
  • Evaluate eligible residents for weight management, including GLP-1 therapy when clinically appropriate.
  • Identify residents who may benefit from follow-up care, including further cognitive review, ongoing medical follow-up, psychiatry, or therapy, and coordinate with their primary care provider.
  • Document every encounter accurately and completely in the EHR according to Medicare standards.
