About The Position
As a Nurse Practitioner in Internal Medicine, provide comprehensive primary care to adult patients in an outpatient setting. Work alongside a collaborative care team to manage acute and chronic conditions, promote preventive care, and build lasting relationships with patients. The position is full time, located at Linden Oaks, in Primary Care - Internal Medicine. The schedule is days, Monday through Friday.
Requirements
- Master of Science in Nursing (required).
- Nurse Practitioner Certification from the NYS Office of Professions.
- Registered Nurse license from the NYS Office of Professions.
- Strong customer service and communication skills.
- Compassionate, warm, and patient-focused approach.
- Ability to work in a stressful environment and take appropriate action.
- Critical thinking skills, decisive judgment, and ability to work with minimal supervision.
- Exceptional documentation skills and professional behavior.
Responsibilities
- Provide comprehensive primary care to adult patients, including evaluating, diagnosing, and managing acute and chronic conditions.
- Develop treatment plans, prescribe medications, order and interpret diagnostic testing, and provide preventive care and patient education.
- Collaborate with physicians, APPs, and other healthcare team members to coordinate care and specialty referrals as needed.
- Maintain timely and accurate documentation of patient visits, diagnoses, and treatment plans.
