About The Position

Conifer Park Outpatient Clinic in Schenectady, NY is seeking a Nurse Practitioner to provide medical care in an outpatient setting. The role includes conducting comprehensive medical histories and examinations, recording initial diagnoses, ordering or administering diagnostic tests, and reviewing and interpreting test results. The Nurse Practitioner assesses the appropriateness of medication-assisted treatments for substance use disorders and provides induction, monitoring, titration, maintenance, and tapering as clinically appropriate.

The position also involves evaluating and treating medical emergencies, referring unstable patients to appropriate facilities, maintaining training in naloxone use and emergency first aid such as CPR, and counseling patients on prescribed treatments. The Nurse Practitioner performs pre-admission clearances and interviews when indicated; develops and implements patient management plans, medication and therapy recommendations, and continuity-of-care plans; and documents follow-up visits, diagnosis and medication updates, and medication effects or side effects. The role includes preparing and educating patients about discharge medications and instructions.

The Nurse Practitioner participates in case assignments and conferences, multidisciplinary treatment team meetings, patient care monitoring conferences, staff and patient educational presentations, medical staff meetings, peer review, and performance-improvement activities. Medical on-call coverage is provided as assigned, along with other duties assigned by the Medical Director. The position offers a sign-on bonus of $7,000 based on 40 hours, prorated based on hours: $3,500 after completing orientation and $3,500 after two years of continuous employment. Background checks, pre-employment screening, and drug screening are required. Requirements

• Postgraduate master's degree in nursing.
• Nursing license to practice in New York State.
• National certification as a Nurse Practitioner, such as through the American Nurses Credentialing Center, National Certification Center, or American Academy of Nurse Practitioners Certification Program.
• CPR certification.
• Unrestricted DEA licensure.
• DATA 2000 waiver.

Nice To Haves

• More than one year of experience as a Nurse Practitioner.
• Experience in addiction medicine or treatment of substance use disorder.
• Active Medicaid number (highly preferred).

Responsibilities

• Conduct comprehensive medical histories and examinations and record initial diagnoses in patient medical records.
• Administer or order diagnostic tests, including x-rays, electrocardiograms, phlebotomy, and other laboratory tests; review and interpret results.
• Assess appropriateness of medication-assisted treatments for substance use disorders and provide induction, monitoring, titration, maintenance, and tapering as clinically appropriate.
• Evaluate and treat medical emergencies and refer unstable patients to appropriate medical facilities.
• Maintain training in naloxone use and emergency first aid response techniques such as CPR.
• Instruct and counsel patients regarding compliance with prescribed therapeutic regimens and treatments.
• Perform pre-admission medical clearances and pre-admission interviews when indicated.
• Develop and implement patient management plans, medication and therapy recommendations, and continuity-of-care plans.
• Perform and document follow-up visits, updating diagnoses and medications as needed.
• Assess and manage medication effects and side effects.
• Participate in case assignments, case conferences, multidisciplinary treatment team meetings, patient care monitoring conferences, educational presentations, and medical staff meetings.
• Provide medical on-call coverage as assigned.
• Participate in peer review activities and performance-improvement initiatives.
• Prepare and educate patients regarding discharge medications and instructions.
• Perform additional duties as assigned by the Medical Director.
