Nurse Practitioner / Physician Assistant
About The Position
San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) is seeking full-time Physician Assistants or Nurse Practitioners. This innovative program serves individuals 55 years or older who have chronic illnesses or disabilities and require coordinated medical care to maintain independence in their homes and communities. The healthcare teams provide a hands-on approach, coordinating medical, social, and home care services. This is a Monday - Friday position at our PACE clinic in Chula Vista, CA. The role involves providing primary care to participants under the clinical direction of the Medical Director. Responsibilities include performing physical assessments, developing and implementing care plans, evaluating and treating physical complaints, and providing participant and caregiver education. The position is a member of the Interdisciplinary Team and requires the ability to assess, plan, and care for frail elder participants according to PACE standards.
Requirements
- Graduate of a school of professional physician assistant or nurse practitioner required.
- Licensed as physician assistant or nurse practitioner in the State of California.
- Certified as a physician assistant practitioner or nurse practitioner by a national certifying organization recognized by the State of California.
- A minimum of one year's experiences in working with the frail and elderly population required.
- Possession of a valid California License as a Physician Assistant or Nurse Practitioner
- Degree from a NCCPA certified program as a Physician Assistant or Nurse Practitioner program
- DEA Number
- Thorough knowledge of current concepts, theories, and practices related to home and community-based care for the elderly and disabled adults.
- Thorough knowledge of physical, mental and social needs of frail older adults.
- Ability to lead and work with the interdisciplinary setting
- Strong organization skills
- Able to manage changing priorities per needs of the PACE program and the agency
- At least one year of experience working with a frail or elderly population
- Must be able to clear fingerprints through DOJ and FBI.
- Must pass a drug screen clearance.
Nice To Haves
- Two years experience as a physician assistant or nurse practitioner in a geriatric setting desirable.
- Working knowledge of the PACE regulations desired.
Responsibilities
- Provides primary care to participants under the clinical direction and supervision of the Medical Director.
- Performs physical assessments of participants.
- Develops and contributes to implementing appropriate plans of care to St. Paul's PACE program participants.
- Evaluates participant’s physical complaints and provides appropriate treatment.
- Provides participant and caregiver teaching and education.
- Participates as a member of the Interdisciplinary Team.
- Assesses, plans, cares for, and provides services to frail elder participants according to assigned responsibilities and PACE standards.
