About The Position

St. Paul's PACE (Program of All-Inclusive Care for the Elderly) provides coordinated medical, social, and home care services to people age 55 and older who live with chronic illness or disabilities, helping them remain as independent as possible in their homes and communities. The healthcare teams take a hands-on approach to coordinating care so participants do not have to manage their medical needs alone. Under the clinical direction and supervision of the Medical Director, the Nurse Practitioner or Physician Assistant provides primary care to participants. The role includes assessing participants, developing and helping implement plans of care, evaluating physical complaints, providing appropriate treatment, and educating participants and caregivers. The clinician also serves as a member of the Interdisciplinary Team and provides services to frail older adults in accordance with assigned responsibilities and PACE standards. This is a full-time, Monday–Friday position at the PACE clinic in Chula Vista, California.

Requirements

Graduate from a professional physician assistant or nurse practitioner program.
Hold a valid California license as a Physician Assistant or Nurse Practitioner.
Be certified as a physician assistant practitioner or nurse practitioner by a national certifying organization recognized by the State of California.
Have a degree from a NCCPA-certified Physician Assistant or Nurse Practitioner program.
Possess a DEA number.
Have at least one year of experience working with a frail or elderly population.
Have training or related experience in the assigned job.
Have thorough knowledge of current concepts, theories, and practices related to home- and community-based care for elderly people and adults with disabilities.
Have thorough knowledge of the physical, mental, and social needs of frail older adults.
Be able to lead and work in an interdisciplinary setting.
Have strong organizational skills.
Be able to manage changing priorities based on the needs of the PACE program and agency.
Clear fingerprint checks through the DOJ and FBI and pass a drug screen.

Nice To Haves

Two years of experience as a Physician Assistant or Nurse Practitioner in a geriatric setting.
Working knowledge of PACE regulations.

Responsibilities

Provide primary care to participants under the clinical direction and supervision of the Medical Director.
Perform physical assessments and develop and contribute to the implementation of appropriate plans of care.
Evaluate participants' physical complaints and provide appropriate treatment.
Provide teaching and education to participants and caregivers.
