About The Position

Salud Family Health is seeking a full-time Nurse Practitioner or Physician Assistant in Trinidad, Colorado. The role uses a team-based care approach to support patients and families in self-management, self-efficacy, and behavior change. The provider performs routine clinic operations, takes patient histories, and conducts physical assessments while adhering to Salud’s mission. Salud provides integrated medical, dental, behavioral health, and clinical pharmacy services, serving individuals regardless of insurance status or ability to pay.

Requirements

Must have graduated from, or be graduating from, an accredited Nurse Practitioner or Physician Assistant program.
Must possess an unrestricted Physician Assistant or Nurse Practitioner license in good standing in the State of Colorado.
Sensitivity to low-income and ethnic minority communities is required.

Nice To Haves

Ability to speak Spanish is strongly preferred.
Three years of experience in a clinical setting as a Nurse Practitioner or Physician Assistant is preferred.

Benefits

Starting annual salary of $118,100 to $151,850.
