Nurse Practitioner/Physician Assistant: Post-Acute & SNF (Full -Time)
About The Position
Theoria Medical is seeking a full-time Advanced Practice Provider to serve residents at local skilled nursing facilities. The role emphasizes continuity of care, building relationships with patients over time, and delivering patient-centered care to vulnerable populations. Providers work with autonomy and flexible scheduling, supported by a collaborative clinical team and technology designed to streamline care and documentation. The position includes six-hour facility shifts for full-time providers, with rounds generally starting between 7 and 10 a.m. There are no on-call or overnight requirements. Providers use a company-provided iPad for charting and have access to an EHR, voice dictation and AI Scribe, and UpToDate clinical guidance. Compensation is productivity-based, with annual performance and longevity bonus opportunities. Theoria Medical also offers benefits and professional development support.
Requirements
- Hold an active U.S. license.
- Nurse Practitioners must be board-certified by ANCC, AANPCB, PNCB, or AACN.
- Physician Assistants must be board-certified by NCCPA.

Responsibilities
- Provide high-quality, patient-centered care to residents in skilled nursing facilities.
- Conduct daily rounding, prescribe medications and treatment plans, and address acute or emergent issues.
- Communicate with facility nursing staff by phone or secure messaging as needed.
- Maintain reliable on-site attendance for scheduled shifts.
- Ensure compliance with clinical standards, facility policies, and regulatory requirements.
