Nurse Practitioner/Physician Assistant: Post-Acute & SNF (Full -Time)
About The Position
Theoria Medical is seeking a full-time Advanced Practice Provider to serve residents at local skilled nursing facilities. The role emphasizes continuity of care, building relationships with patients over time, and delivering high-quality, patient-centered care to vulnerable populations. Providers work with flexibility and autonomy, supported by a collaborative clinical team and technology designed to streamline clinical work. Full-time positions include six-hour facility shifts, with rounds generally starting between 7 and 10 a.m. The role has no on-call or overnight requirements. Providers use a company-provided iPad for charting, an EHR with voice dictation and AI Scribe, and access to UpToDate for clinical guidance. Compensation is productivity-based and includes opportunities for annual performance bonuses and longevity bonuses. Theoria Medical also offers benefits and professional development support.
Requirements
- Hold an active U.S. license.
- Nurse Practitioners must be board-certified by ANCC, AANPCB, PNCB, or AACN.
- Physician Assistants must be board-certified by NCCPA.
- Qualified specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine; other specialties may also qualify.
- Be able to perform the essential functions of the position satisfactorily, with or without reasonable accommodation.
- Undergo criminal background checks and pre-employment drug testing upon acceptance of a contingent offer.
Responsibilities
- Provide high-quality, patient-centered care to residents in skilled nursing facilities.
- Conduct daily rounding.
- Prescribe medications and treatment plans.
- Address acute and emergent issues.
- Communicate with facility nursing staff by phone or secure messaging as needed.
- Maintain reliable on-site attendance for scheduled shifts.
- Ensure compliance with clinical standards, facility policies, and regulatory requirements.
