In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Bridgeport, CT
About The Position
Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations for health plan members. The role focuses on building trusted relationships, improving health outcomes, and helping connect members with care suited to their needs. Visits last approximately 45 minutes and include a medication and medical history review, a physical evaluation, time to answer health questions, and—when ordered by the member’s health plan—additional diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and clinical workflow. They are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, or other airborne allergens or irritants. The role may also involve local travel, travel within licensed states as needed, and virtual visits as needed.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Answer members’ health questions and help connect them with appropriate care.
- Provide Diagnostic & Preventive Services products as indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
