In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Seattle/ Snohomish, WA
About The Position
Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships. The goal is to provide flexible, comprehensive, and personalized health evaluations. The visit, lasting about 45 minutes, includes a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions. Candidates must be comfortable working in diverse home environments, potentially including pets, secondhand smoke, and airborne allergens.
Requirements
- Active, unrestricted license(s) in coverage area(s) SEATTLE SNOHOMISH.
- Board certification.
- Ability to travel locally and within licensed states.
- Ability to conduct virtual visits.
- Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct In-Home Health Evaluations.

- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).
