About The Position

This role is for an experienced LPN/RN looking for an alternative to a large hospital setting, seeking an independent specialty practice environment where teamwork, collaboration, and recognition are valued. The Nurse Care Coordinator, Phone Triage provides clinical specialty care coordination and health advice over the phone with compassion and empathy. They also engage in direct patient care as needed, support daily office operations, and act as a resource for peers to improve patient flow and operational efficiency. This role serves as a liaison between patients, providers, and facility partners, ensuring patients are informed about their care. The practice has 13 Providers and operates in a fast-paced environment, requiring adaptable and agile team members. Center for Urologic Care of Berks County is a rapidly growing, provider-owned organization committed to being an employer of choice, offering a Monday-Friday schedule with no weekends, holidays, or on-call duties.

Requirements

Graduate of an accredited School of Nursing
Current Pennsylvania LPN or RN License
BLS Certification
Warm, engaging, optimistic attitude
Willingness to learn, collaborate and grow as a human
Passionate about communicating with and advocating for patients

Responsibilities

Performs clinical specialty care coordination to patients.
Provides health advice over the phone with compassion and empathy.
Engages in direct patient care as needed throughout the clinical office visit experience.
Supports the daily operations of patient flow in the office.
Acts as a vital resource for peers at all levels to minimize delays in care and improve operational inefficiencies.
Serves as a liaison for patients, providers, and facility partners, ensuring patients are comfortable and informed about their care.
