About The Position

The Home Delivered Meals Program (HDMP) Coordinator is responsible for overseeing the delivery of Meals-on-Wheels by drivers. This includes ordering the correct number of meals from caterers, ensuring compliance with sanitary and health regulations, and making sure the program meets all agency and funding source requirements. The coordinator establishes systems to ensure drivers deliver meals to the homebound according to regulations, reviews the STARS program daily for client updates, and maintains daily contact with drivers regarding route changes and client status. They also advise Case Managers of client issues, handle client complaints, respond to "No Answers" reported by drivers, and process client contributions according to NYC Aging and agency policies. Additionally, the coordinator provides training and annual route observations for drivers, ensures driver personnel files are up-to-date, prepares documents for annual permits, and submits staff travel reimbursement forms weekly. A monthly statistical report is prepared by the last day of each month, ensuring meal unit entries match the NYC Aging database. The role oversees client meal/menu preferences, creates monthly menus, participates in food ordering, and develops customer satisfaction surveys. The coordinator may be assigned to assist at multiple HDM sites, help with deliveries if necessary, observe meal packaging for temperature compliance, provide driver training twice a year, and submit meal reports to the Program Director upon request.

Requirements

  • High School Diploma
  • NYC DOHMH Food Handlers Certification
  • Valid New York State Driver’s License (Class D) and provision of printout of annual Driving Record from Motor Vehicle Bureau.
  • One (1) year driving experience with no more than one moving violation within a 24 month period, no driving while intoxicated convictions.

  • Able to travel to different locations to caterers

Responsibilities

  • Oversee the delivery of Meals-on-Wheels by drivers.
  • Order the number of lunches from the caterer.
  • Maintain proper sanitary and health regulations.
  • Ensure program meets all agency and funding source regulations.
  • Set up systems to ensure Drivers deliver meals to the homebound according to funding source regulations.
  • Review the STARS program daily for any new intakes, terminations or client changes.
  • Maintain route sheets daily to include cancellations and additions of clients.
  • Maintain daily contact with Drivers regarding route changes, clients’ cancellations or additions to the MOWs routes.
  • Advise Case Managers of client problems reported by clients or Drivers.
  • Handle client complaints and changes and report them to Program Director.
  • Respond according to agency regulations to “No Answers” reported by drivers.
  • Collect and process clients’ contributions according to NYC Aging and agency policies.
  • Provide training and observe the drivers by accompanying them on their routes annually.
  • Ensure that the drivers’ personnel files contain current licenses, registrations, and insurance cards.
  • Prepare documents to secure annual permits for the drivers.
  • Prepare and submit staff travel reimbursement forms for the drivers weekly.
  • Prepare monthly statistical report by the end of the last day of each month.
  • Ensure meal unit entries from route sheets accurately match the NYC Aging Peer Place/STARS database.
  • Oversee client meal/menu preferences (including new ethnic meal) and creates monthly menus for clients at CCNS Home Delivered Meals Programs.
  • Participate in placing food orders to caterers for CCNS Home Delivered Meals Programs.
  • Create meal and customer satisfaction surveys for home delivered meal clients and tally’s the results to present findings to the Managers and the Director of Field Operations.
  • Assist with deliveries when necessary.
  • Observe packaging of meals, ensure temperature of food is consistent with DFTA regulations.
  • Provide training and document it twice a year to drivers.
  • Provide reports of meals to Program Director, when requested.
