About The Position

The Home Delivered Meals Program (HDMP) Coordinator is responsible for overseeing the delivery of Meals-on-Wheels by drivers. This includes ordering the correct number of lunches from caterers, maintaining proper sanitary and health regulations, and ensuring the program adheres to all agency and funding source regulations. The role involves setting up systems for timely meal delivery to homebound clients, managing route sheets with daily changes, and maintaining communication with drivers regarding route adjustments and client status. The coordinator also advises Case Managers on client issues, handles client complaints, and responds to "No Answers" reported by drivers. Additionally, the position involves collecting client contributions, providing annual training and observation for drivers, ensuring driver personnel files are up-to-date, preparing necessary documents for permits, and submitting staff travel reimbursement forms. The role also requires preparing monthly statistical reports, verifying meal unit entries against the database, overseeing client meal/menu preferences, participating in food orders, and creating/analyzing customer satisfaction surveys. The coordinator may assist at multiple sites and help with deliveries when needed, observing meal packaging and food temperature to ensure compliance with regulations. Training and documentation for drivers are required twice a year, and reports are provided to the Program Director upon request.

Requirements

  • High School Diploma
  • NYC DOHMH Food Handlers Certification
  • Valid New York State Driver’s License (Class D) and provision of printout of annual Driving Record from Motor Vehicle Bureau.
  • One (1) year driving experience with no more than one moving violation within a 24 month period, no driving while intoxicated convictions.

Responsibilities

  • Set up systems to ensure Drivers deliver meals to the homebound according to funding source regulations.
  • Review the STARS program daily for any new intakes, terminations or client changes to ensure the senior clients’ needs are met expeditiously.
  • Maintain route sheets daily to include cancellations and additions of clients.
  • Maintain daily contact with Drivers regarding route changes, clients’ cancellations or additions to the MOWs routes.
  • Advise Case Managers of client problems reported by clients or Drivers.
  • Handle client complaints and changes and report them to Program Director.
  • Respond according to agency regulations to “No Answers” reported by drivers.
  • Collect and process clients’ contributions according to NYC Aging and agency policies.
  • Provide training and observe the drivers by accompanying them on their routes annually.
  • Ensure that the drivers’ personnel files contain current licenses, registrations, and insurance cards.
  • Prepare documents to secure annual permits for the drivers.
  • Prepare and submit staff travel reimbursement forms for the drivers weekly.
  • Prepare monthly statistical report by the end of the last day of each month.
  • Ensure meal unit entries from route sheets accurately match the NYC Aging Peer Place/STARS database.
  • Oversees client meal/menu preferences (including new ethnic meal) and creates monthly menus for clients at CCNS Home Delivered Meals Programs.
  • Participates in placing food orders to caterers for CCNS Home Delivered Meals Programs.
  • Creates meal and customer satisfaction surveys for home delivered meal clients and tally’s the results to present findings to the Managers and the Director of Field Operations.
  • Coordinator may be assigned to assist at multiple Home Delivered Meal sites.
  • Assist with deliveries when necessary.
  • Observe packaging of meals, ensure temperature of food is consistent with DFTA regulations.
  • Provide training and document it twice a year to drivers.
  • Provide reports of meals to Program Director, when requested.
