About The Position

NCSS is seeking a Care Coordinator to provide integrated, person-centered support to military members, veterans, and their families. The coordinator helps ensure timely, coordinated care that reflects each person’s goals and needs by collaborating with healthcare providers, community agencies, and the Department of Veterans Affairs. The role supports access to appropriate care, navigation of benefits, and smooth transitions between services.

The coordinator supports ongoing case management, administrative and documentation processes, quality improvement initiatives, and communication with clients and partners to promote engagement and wellness. The work addresses needs such as trauma recovery, reintegration challenges, housing stability, and behavioral health coordination. The position calls for flexibility, strong communication, and a commitment to trauma-informed, culturally competent care.

Requirements

• Bachelor’s degree preferred, or an equivalent combination of related education, training, and experience.
