About The Position

Merakey seeks a Part-Time Mental Health Clinic Case Manager to join their Behavioral Health Urgent Care (BHUCC) team in Philadelphia, Pa. This role involves providing urgent care for individuals experiencing emotional or behavioral health concerns, offering resolution-focused services and aftercare planning. The goal is to help individuals access mental health care when needed, reducing the need for crisis center and emergency room visits. The position serves individuals regardless of income or insurance status.

Requirements

• Bachelor’s degree in social work, Psychology, Counseling, or a related field.
• Previous experience in case management, social work, or related roles, preferably in behavioral health or crisis settings.
• Knowledge of public and private benefit programs and community resources related to Social Determinants of Health (SDOH).
• Excellent communication and interpersonal skills to engage effectively with individuals and colleagues.
• Empathy, compassion, and a person-centered approach.
• Ability to work collaboratively in a multidisciplinary team environment.
• Strong organizational and time management skills to manage case management responsibilities effectively.
• Familiarity with electronic health records (EHR) systems and healthcare technology.

Responsibilities

• Provides immediate and follow-up case management services to individuals experiencing urgent behavioral health challenges.
• Offers comprehensive support, including benefit enrollment, information about external resources addressing the Social Determinants of Health, referrals, and completing outreach contact calls.
• Utilizes a collaborative and empathetic approach to help individuals access necessary resources for their health needs and overall well-being.
