About The Position

The Mental Health Intake Nurse provides onboarding for patients admitted to inpatient units and support to inpatient units during patient discharges. The nurse assesses, plans, implements, evaluates, and documents all aspects of patient care, working with healthcare providers to coordinate efficient, effective, quality care. The role requires knowledge of growth and development across the appropriate lifespan, the ability to assess and interpret patient information and age-specific needs, and the ability to provide care appropriate to the patient population. The nurse follows unit, department, and institutional policies, procedures, and standards. The nurse coordinates care with physicians and collaborates with unit staff, program staff, and other departments. Responsibilities include medication administration and reconciliation, patient and family education, behavioral and safety interventions, treatment planning with the interdisciplinary team, and accurate documentation of care and patient responses. The nurse also supports new-patient onboarding and discharge processes.

Requirements

Registered Nurse (RN).
Registered Nurse Diploma.
Basic Cardiac Life Support.
Nonviolent Crisis Intervention certification must be obtained within 60 days of employment start date.
Knowledge of growth and development across the appropriate life span and ability to assess age-specific patient needs.
Ability to provide care appropriate to the relevant patient group or groups.
Ability to comply with unit, department, and institutional policies, procedures, and standards.

Responsibilities

Provide an in-depth onboarding experience for patients admitted to inpatient units.
Support inpatient unit staff with patient discharges.
Assess, plan, implement, evaluate, and document all aspects of patient care.
Coordinate care with physicians and collaborate with other units, program staff, and departments to support optimal patient care.
Provide patient education while administering medication and use consistent patient and medication scanning through eMar.
Reconcile medications on admission, during transfers, and at discharge according to facility guidelines.
Assign and conduct individual and group interventions in the milieu for behavioral and safety management.
Maintain appropriate boundaries and use least restrictive interventions appropriately.
Collaborate with the interdisciplinary team to assess, plan, implement, and evaluate treatment, patient safety, and programs.
Assess and document patients’ responses to treatment and safety interventions.
Help patients and families understand diagnoses, prescribed treatment, and safe symptom management for specific mental disorders.
Document care against interventions in the Recovery Plan and provide psychoeducational and medical teaching related to patient safety needs.
Renew nursing license and complete mandatory education, health screening, CPR, and CPI requirements within designated time frames, providing written verification.
Transcribe orders accurately and scan orders to the pharmacy promptly.
