About The Position

The Memory Care Support Partner at Vitalia Westlake helps residents through daily life and creates human connection, particularly during vulnerable moments. The role involves learning residents’ interests and abilities, collaborating on engagement plans, and implementing those plans patiently. The Support Partner prepares and arranges resident and shared spaces for daily routines and activities, partners with residents from waking through bedtime, documents care and changes in condition, and helps orient team members assigned to memory care neighborhoods. This is a full-time position with a two-week rotating schedule: Tuesday, Thursday, and Friday in week one; Monday, Saturday, and Sunday in week two. Shifts are 7:00 a.m. to 7:00 p.m. The stated pay range is $18 to $24.25 per hour.

Requirements

Be at least 18 years of age.
Have a high school diploma or equivalent.
Be able to read, write, understand, and communicate in English with residents.
Have a positive and energetic attitude and a willingness to care for residents.
Possess computer skills sufficient to print documents and enter resident information.
Be able to stand, walk, bend, kneel, and stoop throughout the day.
Be able to frequently lift and/or move items weighing up to 50 pounds.
Be able to assist residents with sitting, standing, and walking, including after a fall.
Be criminally cleared.

Responsibilities

Create human connection with residents and approach their care with empathy.
Observe and learn each resident’s interests and abilities, and collaborate on appropriate engagement plans.
Implement engagement strategies patiently.
Prepare resident rooms and shared areas for meals, events, daily activities, and nighttime routines.
Partner with residents in daily life from waking through bedtime.
Document resident care and changes in condition.
Assist with orientation of team members assigned to memory care neighborhoods.
Assist residents with sitting, standing, and walking, including assisting people after a fall.
