About The Position

The Memory Care Support Partner creates human connections with residents and supports their daily lives in a memory care neighborhood. The role involves observing residents’ interests and abilities, collaborating on appropriate engagement plans, and patiently implementing strategies. The Support Partner prepares resident and shared spaces for daily activities and routines, partners with residents throughout the day, documents care and changes in condition, and assists with orienting team members assigned to memory care neighborhoods. This is a full-time overnight position at Vitalia Westlake in Westlake, Ohio. The schedule is 7:00 p.m. to 7:00 a.m.: Week 1, Tuesday, Thursday, and Friday; Week 2, Monday, Saturday, and Sunday. Pay is $18–$24.25 per hour.

Requirements

Be at least 18 years of age.
Have a high school diploma or equivalent.
Be able to read, write, understand, and communicate in English with residents.
Have a positive and energetic attitude and a strong commitment to residents.
Have computer skills sufficient to print documents and enter resident information.
Be able to stand, walk, bend, kneel, and stoop throughout the day.
Be able to frequently lift and/or move items weighing up to 50 pounds.
Be criminally cleared.

Responsibilities

Create empathetic human connections with residents, including during vulnerable moments.
Observe residents’ interests and abilities and collaborate on appropriate engagement plans and strategies.
Prepare dining, activity, resident, and other spaces to support residents’ daily experiences and routines.
Partner with residents in their daily activities from waking through bedtime.
Document resident care and changes in condition.
Assist with orientation of team members assigned to memory care neighborhoods.
Assist residents with sitting, standing, and walking, and assist persons after a fall.
