About The Position

The Memory Care Support Partner works at Vitalia Highland Heights in Highland Heights, Ohio, supporting residents in a memory care setting. The role centers on building empathetic human connections, learning about residents’ interests and abilities, and helping create and carry out appropriate engagement plans. The Support Partner prepares spaces and residents for daily activities and routines, and partners with residents throughout the day to support daily life. The role also includes documenting resident care and changes in condition and assisting with the orientation of team members assigned to memory care neighborhoods. This is a full-time position on a 7:00 a.m. to 7:00 p.m. schedule. Week 1 shifts are Tuesday, Thursday, and Friday; Week 2 shifts are Monday, Saturday, and Sunday. Pay is $18–$24.25 per hour.

Requirements

Be at least 18 years of age.
Have a high school diploma or equivalent.
Be able to read, write, understand, and communicate in English with residents.
Have a positive, energetic attitude and a strong interest in residents.
Have computer skills sufficient to print documents and enter resident information.
Be able to stand, walk, bend, kneel, and stoop throughout the day.
Be able to frequently lift and/or move items weighing up to 50 pounds.
Be able to assist residents with sitting, standing, and walking, and to assist people after a fall.
Receive criminal clearance.

Responsibilities

Create human connections with residents through empathy, particularly during vulnerable moments.
Observe and learn residents’ interests and abilities, collaborate on suitable engagement plans, and implement strategies patiently.
Prepare dining, activity, and resident spaces to support residents’ daily experiences and routines.
Partner with residents to support daily life from waking through bedtime.
Document resident care and changes in condition.
Assist with orienting team members assigned to memory care neighborhoods.
