About The Position

The Memory Care Support Partner creates human connections with residents during vulnerable moments and learns about residents’ interests, abilities, and individual identities. The role collaborates on appropriate engagement plans and patiently implements strategies to support residents’ success. The Support Partner prepares and maintains environments for residents’ daily experiences, from dining and events to nighttime routines, and partners with residents throughout the day to support daily life. The role also documents resident care and changes in condition and assists with orienting team members assigned to memory care neighborhoods.

Requirements

Must be at least 18 years of age.
High school diploma or equivalent.
Ability to read, write, understand, and communicate in English with residents.
Positive and energetic attitude and enthusiasm for working with residents.
Computer skills sufficient to print documents and enter resident information.
Must be active and able to stand, walk, bend, kneel, and stoop throughout the day.
Ability to frequently lift and/or move items weighing up to 50 pounds.
Must be able to assist residents with sitting, standing, and walking, as well as assist persons after a fall.
Must be criminally cleared.

Responsibilities

Create empathetic human connections with residents.

Prepare resident spaces and shared areas for daily activities and routines, including meals, events, and nighttime routines.
Partner with residents to support all facets of daily life.
Document resident care and changes in condition.
Assist with orientation of team members assigned to memory care neighborhoods.
Assist residents with sitting, standing, and walking, including assisting persons after a fall.
