About The Position

The Memory Care Support Partner works with residents in a memory care neighborhood to support daily life, resident engagement, and quality of care. The role centers on empathetic human connection, observing and understanding residents’ interests and abilities, and collaborating on appropriate engagement plans. The Support Partner helps create a welcoming environment throughout the day, from preparing dining and activity spaces to supporting residents’ nighttime routines. The position also involves partnering with residents in daily activities, documenting care and changes in condition, and assisting with the orientation of team members assigned to memory care neighborhoods. This is a full-time position at Vitalia Solon in Solon, Ohio. The posted pay range is $18–$24.25 per hour. The schedule is 7:00 a.m. to 7:00 p.m., with Monday, Saturday, and Sunday in Week 1 and Tuesday, Thursday, and Friday in Week 2.

Requirements

Be at least 18 years of age.
Have a high school diploma or equivalent.
Be able to read, write, understand, and communicate in English with residents.
Have a positive and energetic attitude and a strong interest in working with residents.
Have computer skills sufficient to print documents and enter resident information.
Be able to stand, walk, bend, kneel, and stoop throughout the day.
Be able to frequently lift and/or move items weighing up to 50 pounds.
Be able to assist residents with sitting, standing, and walking, and assist persons after a fall.
Successfully complete criminal clearance.

Responsibilities

Create empathetic human connections with residents, particularly during vulnerable moments.
Observe residents to learn their interests, abilities, and individual preferences.
Collaborate on appropriate resident engagement plans and implement strategies patiently.
Prepare and arrange resident spaces and shared environments for daily activities and routines.
Partner with residents in daily life from waking through bedtime.
Document resident care and changes in condition.
Assist with orienting team members assigned to memory care neighborhoods.
