About the position
The Membership Growth Consultant is responsible for educating and enrolling patients into a comprehensive care program designed for individuals with kidney conditions. They will proactively reach out to patients via phone, build trusting relationships, and educate them on the benefits of enrolling with Strive Care365. The consultant will also assist patients in navigating insurance questions, visit patients in-person when possible, and maintain long-term relationships to support patient retention and satisfaction. Additionally, they will meet with internal and external stakeholders to facilitate business priorities and opportunities.
Responsibilities
- Proactively outreach to patients via phone to educate them on the care services available and enroll them
- Build trusting, long-term relationships with patients and educate them on the requirements to enroll with Strive Care365
- Meet or exceed daily outreach expectations towards outbound calls, connections, and enrollments
- Present oneself as an educational and empathetic resource to potential patients about the care model
- Assist patients with navigating confusion about the care model, Medicare benefits, and how it differentiates from their current care
- Have knowledge of Medicare and apply it to help patients navigate insurance questions and issues
- Visit with patients in-person, when possible, at home or during medical visits
- Refer patients to approved licensed insurance broker partner when patient interest is expressed
- Meet with partner nephrologists and practice staff to build credibility of Strive Care365 and earn the right to meet patients at their locations
- Maintain up-to-date client information in customer relationship management (CRM) tool
- Maintain long-term relationships with patients to positively support patient retention and satisfaction
- Meet in person with internal and/or external stakeholders to facilitate team and business priorities/opportunities
- Have a positive and proactive attitude towards building a business
- Perform other duties as assigned
Requirements

- Have a positive and proactive attitude towards building a business
- High School Diploma
- 2+ years' experience working with customers or patients in a professional setting
- Experience with promoting and selling primary care or other healthcare related services
