About The Position
We are seeking a PRN Medical Social Worker (MSW) to serve patients in Orange, Osceola, Seminole, and Brevard counties. The role helps patients and families navigate emotional, social, and practical challenges during treatment and recovery. The position is part-time and involves home health work on the road, generally Monday through Friday, with weekends as needed. The role requires screening through the Care Provider Background Screening Clearinghouse.
Requirements
- Master’s degree in social work (MSW) from an accredited institution.
- Current Florida licensure as an LCSW.
- At least one year of experience in a medical or home health setting.
- Strong understanding of community resources and healthcare systems.
- Excellent communication, documentation, and organizational skills.
- Ability to work independently and manage a flexible schedule.
- Compassionate, patient-centered approach.
Responsibilities
- Conduct psychosocial assessments to determine patient needs and develop individualized care plans.
- Provide counseling and emotional support to patients and families.
- Assist with access to community resources, financial assistance programs, and long-term care planning.
- Collaborate with the interdisciplinary healthcare team to ensure comprehensive patient care.
- Support discharge planning and coordination of services.
- Maintain accurate and timely documentation in compliance with agency and regulatory requirements.
