About The Position

AccordCare Home Health, a licensed Medicare-certified home health agency, is seeking a full-time Field Registered Nurse to provide high-quality Medicare home health care to patients in Brevard County, Florida. The nurse provides care based on individualized Plans of Care, documents services and communicates with patients, families, and the interdisciplinary team. This role requires screening through the Care Provider Background Screening Clearinghouse.

Requirements

• Current Registered Nurse license in the State of Florida.
• Experience in Medicare home health.
• Wound care and infusion skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively and provide excellent customer service.
• Proficiency with electronic health records for documentation and communication.
• Screening through the Care Provider Background Screening Clearinghouse is required.

Nice To Haves

• At least one year of home health experience is preferred.
• WellSky/Kinnser experience is a plus.

Responsibilities

• Provide safe and effective nursing care based on individualized Plans of Care.
• Administer medications and provide wound care, infusions, catheter care, and other nursing tasks as required.
• Promptly communicate changes in a patient’s medical status or emergencies to the team for effective case management.
• Assist with patient transfers, positioning, and mobility needs.
• Maintain accurate documentation in the electronic health record.
• Collaborate with patients, families, and interdisciplinary team members.
• Demonstrate empathy and attention to detail, and prioritize tasks effectively.
