Medical Care Manager

AIDS Healthcare Foundation•Fort Myers, FL

About The Position

The Medical Care Manager (MCM) combines AIDS Healthcare Foundation’s mission with Medical Care Management, applying relevant experience, skills, education, and knowledge of the Ryan White Care Act Program. Working within a Healthcare Center alongside providers and the healthcare team, the MCM supports Ryan White patients in understanding program eligibility and accessing available resources. The role includes meeting with patients, completing Health Risk/Needs Assessments, developing Service/Action/Care Plans, and providing ongoing education about diagnoses and medications. For Ryan White contracts that include psychosocial needs, the MCM addresses mental health and basic needs such as housing, food, and clothing. The MCM builds relationships with relevant programs and agencies and facilitates referrals, support, and assistance for patients lacking those services. Using case management skills, the MCM meets patients where they are, identifies strengths and opportunities for growth, and helps them set realistic goals. The MCM collaborates with patients and providers, often serving as the liaison between them and helping patients put plans of care into action. Through assessments, plans, referrals, support, and brief interventions focused on achievable long-term goals, the MCM contributes to Ryan White contract deliverables, HIV/AIDS Bureau measures, and Department of Health and Human Services measures. The work supports patients’ progress toward viral suppression, better health outcomes, and improved quality of life. AIDS Healthcare Foundation is a specialized provider of HIV/AIDS medical care.
