About The Position

Lifespace is hiring a full-time Licensed Vocational Nurse (LVN) for the Memory Care Department at Meadow Lake in Tyler, Texas. The role is scheduled for the 2 PM to 10 PM shift and pays $25.24–$34.73+ per hour. The LVN works with an interdisciplinary team to coordinate and evaluate resident care, oversee nursing assistant assignments, administer medications and treatments, maintain accurate documentation, and support communication with residents’ families. The position contributes to individualized care planning and coordinates resident admissions, discharges, and transfers.

Requirements

• Graduate of an accredited Licensed Practical Nurse (LPN) program.
• Licensed Practical Nurse license, certifications, and other licensure required by state regulations.

Responsibilities

• Participate with the interdisciplinary team to review, plan, coordinate, and evaluate resident care.
• Document residents’ conditions and nursing needs accurately and promptly, and report pertinent observations and reactions in a timely manner.
• Develop, direct, and monitor nursing assistant assignments, adjusting them based on census and required level of care.
• Execute treatments as necessary and document reactions to medications and treatments.
• Generate, prepare, administer, and chart medications.
• Initiate physician orders.
• Communicate with families regarding medication changes and changes in residents’ conditions.
• Establish protocols to ensure care plans are continually reviewed and contain updated, accurate information reflecting each resident’s individuality.
• Coordinate admissions, discharges, and transfers to deliver quality customer service.
