LVN Memory Care
About The Position
The LVN Memory Care position is in the Memory Care Department at Meadow Lake in Tyler, Texas, and is a full-time role on the 2 PM to 10 PM shift. The nurse works with the interdisciplinary team to plan, coordinate, evaluate, and deliver resident care. Duties include documenting residents’ conditions and nursing needs; monitoring and reporting observations and reactions; directing nursing assistant assignments; administering and documenting treatments and medications; initiating physician orders; communicating with families about medication or condition changes; keeping care plans accurate and individualized; and coordinating admissions, discharges, and transfers.
Requirements
- Graduate of an accredited Licensed Practical Nurse program.
- Licensed Practical Nurse license and certifications, as well as any other licensure required by state regulations.
Responsibilities
- Participate with the interdisciplinary team to review, plan, coordinate, and evaluate residents’ care.
- Accurately and promptly document residents’ conditions and nursing needs.
- Report pertinent observations and reactions regarding residents in a timely manner.
- Develop, direct, and monitor nursing assistant assignments, adjusting them based on census and level of care required.
- Execute treatments as necessary and document reactions to medications and treatments.
- Generate, prepare, administer, and chart medications.
- Initiate physician orders.
- Facilitate communication with families regarding medication changes and changes in residents’ conditions.
- Establish protocols to ensure care plans are continually reviewed and contain updated, accurate information reflecting each resident’s individuality.
- Coordinate admissions, discharges, and transfers to deliver quality customer service.
