About The Position

White Blossom Care Center is seeking an experienced LVN Supervisor to join its leadership team at a skilled nursing facility in San Jose, California. The role oversees nursing operations on assigned shifts, ensuring continuity of resident care and compliance with applicable regulations. The LVN Supervisor contributes to the interdisciplinary team by managing admissions, conducting clinical audits, and mentoring frontline staff. Direct skilled nursing facility experience is required; new graduates are not being considered.

Requirements

• Current and active California LVN license in good standing.
• Direct skilled nursing facility experience.
• Proficiency with PointClickCare (PCC), specifically performing audits.
• Strong understanding of Title 22 and long-term care regulations.
• Excellent communication, leadership, and organizational abilities in a fast-paced setting.

Nice To Haves

• One to two years of supervisory or charge nurse experience preferred.

Responsibilities

• Supervise and support LVNs and CNAs, providing guidance and performance feedback.
• Coordinate and complete resident admissions and associated clinical documentation.
• Conduct and oversee PointClickCare (PCC) audits to ensure accuracy, clinical integrity, and compliance.
• Ensure nursing staff on the assigned shift adhere to state, federal, and facility regulations.
• Monitor resident conditions and collaborate with the interdisciplinary team to update care plans.
• Assist with staffing and scheduling.
• Help maintain a safe, efficient, and compassionate environment.
