About The Position

Coordinates the integration of social services and case management functions into patient care, discharge, and home planning processes. Coordinates with external service organizations, agencies, and healthcare facilities. The role manages patients across the continuum of care and supports coordination of care, discharge planning, and continuing treatment.

Requirements

• Experience in public health nursing, acute care, case management, and/or in-home health care.
• Knowledge of disease management strategies.
• Knowledge of nursing processes, care management, and continuity of care.
• Ability to identify and resolve problems in a timely manner, gather and analyze information skillfully, and develop alternative solutions.
• Ability to maintain strict protocols for confidential or sensitive information.
• Computer proficiency and working knowledge of Microsoft Office Suite.
• Strong verbal, written, and interpersonal communication skills.
• High school diploma or GED required.
• Three years of nursing experience required.
• Active LVN certification required.
• Active CPR/BLS required.

Responsibilities

• Acts as a liaison between patients and families and healthcare personnel to ensure necessary care is provided.
• Manages patients throughout the continuum of care.
• Performs utilization review to assess the appropriateness of admissions and medical necessity.
• Coordinates and oversees patient care and services to achieve desired clinical and resource outcomes.
• Coordinates with physicians regarding patient progress and discharge needs.
• Works with discharge planners and assists in coordinating support services.
• Educates and supports patients and families about medical conditions, treatments, and the importance of following continuing treatment plans.
• Advocates for patients when dealing with healthcare providers, insurance plans, and community resources.
• Performs other duties as assigned.
