About The Position

The full-time Case Manager (LPN/LVN) oversees and manages the delivery of care for residents from pre-admission through post-admission. The role coordinates services and care utilization so residents and their families receive appropriate support and high-quality care. The Case Manager works with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans, monitor resident progress, facilitate transitions of care, and advocate for residents and families.

Requirements

Successfully graduated from an approved and certified LVN program.
Hold a current, unencumbered license to practice as an LVN in the state.
Ability to make quick judgments and work independently.
Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines pertaining to long-term care.

Nice To Haves

Two years of experience as an LVN/LPN.
Active patient or bedside care experience within the past three years.

Responsibilities

Manage resident care throughout the care continuum, including pre-admission, admission, and post-admission.
Coordinate services to meet residents’ needs in a timely and effective manner.
Collaborate with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans based on resident needs.
Monitor resident progress and ensure appropriate interventions are implemented when necessary.
Facilitate transitions of care and ensure services and resources are in place for each resident.
Advocate for residents and families, ensure their needs are met, and provide education as needed.
