About The Position
Albany Med Health System is seeking a motivated and energetic Licensed Practical Nurse to join its General Obstetrics outpatient practice and Women's Health team at Columbia Memorial Hospital in Hudson, New York. The practice offers a friendly, team-oriented work environment focused on efficient, compassionate patient care. Under the direction of an attending physician, RN, Lead LPN, or practice coordinator, the LPN provides direct patient care and assists providers with clinical functions. The role may require access to sensitive health system and patient information, including HIPAA-protected health information. Workforce members must limit access, use, and disclosure to the minimum necessary for their assigned duties and protect information from unauthorized access and modification in accordance with health system policies and standards.
Requirements
- High school diploma or GED.
- Active LPN state licensure upon hire.
- Excellent verbal and written communication skills.
- Ability to effectively present information and respond to questions from physicians, patients, family members, and other employees in the work setting.

Responsibilities
- Obtain and accurately record patients’ vital signs.
- Document components of patients’ histories.
- Communicate openly and appropriately with patients, visitors, and staff.
- Participate in in-office procedures under the direction and supervision of a licensed healthcare provider.
- Perform diagnostic CLIA-waived testing.
- Perform venipuncture to obtain blood for laboratory testing.
- Conduct prescription call-backs within the scope of practice.
- Prepare patients’ charts with pertinent clinical information.
- Maintain supply inventory and keep patient examination rooms stocked.
- Adhere to Albany Med Center regulatory compliance requirements.
- Protect sensitive information by limiting access, use, and disclosure to the minimum necessary and following applicable policies and standards.
