About The Position
As a Licensed Practical Nurse at American Senior Communities, you will provide compassionate, person-centered care to residents and help support their physical and psychosocial well-being. The role involves developing meaningful relationships with residents and staff, providing hands-on nursing care, monitoring health, and contributing to individualized care plans. Weekend Option shifts are 6 p.m. to 6 a.m. on Friday, Saturday, and Sunday.
Requirements
- Hold a current, valid Licensed Practical Nurse license in the state of Indiana.
- Have proficient medication management skills.
- Be able to conduct thorough assessments and accurately document changes in residents’ conditions.
- Have a strong passion for geriatric nursing and a commitment to senior care excellence.
- Have strong communication and interpersonal skills.
Responsibilities
- Deliver exceptional nursing care to residents, supporting their physical and emotional well-being.
- Provide hands-on care, including administering medications, wound care, and monitoring vital signs.
- Monitor and assess the health of residents with a variety of highly acute needs.
- Conduct resident health assessments, document changes, and communicate with the healthcare team.
- Assist residents with daily tasks while fostering independence and supporting daily activities.
- Contribute to the development and implementation of individualized care plans that address each resident’s needs.
- Engage in meaningful interactions and provide detail-oriented, person-centered care.
