LPN - Home Health Care (Carlisle)
About The Position
Helping Hand Nurse, LLC, a home health care agency providing medical and non-medical services, is seeking a Licensed Practical Nurse to provide one-to-one care for pediatric patients in their homes. The role follows the clinical team's directions and the patient's plan of care, supports patient and family teaching, communicates clinical changes to the physician, and responds promptly and accurately to changes in the patient's condition or treatment response. Available shifts include days, evenings, and overnights. The employer is willing to accommodate employees' schedules and offers part-time or full-time hours.
Requirements
- Hold an LPN license.
- Have a clean background, criminal record, and child abuse background check with fingerprints.
- Be CPR certified.
- Have knowledge of computerized documentation and office software.
Responsibilities
- Provide one-to-one care in the patient's home.
- Take directions from the clinical team.
- Follow the plan of care.
- Direct and guide patient and family teaching and activities while maintaining standards of care.
- Communicate with the physician about changes in the patient's clinical condition.
- Respond quickly and accurately to changes in the patient's condition or response to treatment.
- Follow all clinical and office policies.
