LPN / RN Home Health Nurse
About The Position
Angels of Care Home Health is seeking Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide direct, one-on-one medical care to infants, children, and/or adolescents in their homes in Surprise, Arizona, and surrounding areas. Nurses provide care according to the active plan of care established by the physician and other appropriate personnel. Full-time and part-time shifts are available, including days, nights, and weekends. New graduates are welcome, and training is provided. Angels of Care has served the medically fragile community nationwide since 2000. Its stated mission is to provide children and young adults in need with high-quality home health care in a loving, caring, and professional manner. Requirements
- Active RN or LPN/LVN license.
- Ability to provide care in a client home setting.
- Ability to make a positive and lasting impression.
Responsibilities
- Provide direct, one-on-one care to patients in their homes according to the active plan of care.
- Conduct ongoing patient care and assessments.
- Administer prescribed medications, treatments, and therapies.
- Coordinate care.
- Educate family members on the patient's clinical care to support positive outcomes.
- Carry out preventative initiatives to protect the quality of patient care.
