About The Position

Angels of Care Home Health is seeking Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide direct, one-on-one home health care to medically fragile infants, children, and adolescents in Glendale, Arizona, and surrounding areas. Nurses provide care in patients’ homes according to the active plan of care established by the physician and other appropriate personnel. Full-time and part-time shifts are available, including days, nights, and weekends. Angels of Care has served the medically fragile community since 2000. Its mission is to provide children and young adults in need with high-quality home health care in a loving, caring, and professional manner. The organization emphasizes patient-centered care, advocacy, employee support, outreach, and responsive service. New graduates are welcome, and training is provided.

Requirements

Active RN or LPN/LVN license.
Ability to provide care in a client’s home setting.

Responsibilities

Provide direct, one-on-one care to infants, children, and/or adolescents primarily in their homes, following the active plan of care.
Conduct ongoing patient care and assessments.
Administer prescribed medications, treatments, and therapies.
Coordinate patient care.
Educate family members about the patient’s clinical care to support positive outcomes.
Use preventative initiatives to protect the quality of care.
