About The Position

Concordia of Monroeville is seeking a Resident Care Coordinator for its Personal Care unit. The Unit Manager / Resident Care Coordinator is responsible for ensuring high-quality care, compliance, and customer service standards on their assigned unit(s). This role focuses on meeting the medical, physical, social, and emotional needs of residents, with at least 75% of time spent on the floor providing direct care and oversight.

Requirements

  • Licensed Practical Nurse with a valid current Pennsylvania license
  • Excellent communication skills (verbal and written)
  • Knowledgeable of Department of Human Services personal care home regulations
  • Point Click Care Experience

Responsibilities

  • Read daily reports and follow up to ensure resident care issues are addressed.
  • Review the doctor list daily and follow up to ensure resident care issues are addressed.
  • Participate in weekly care planning meetings.
  • Complete initial and annual PCH Assessments using the correct DHS-required form in a timely manner.
  • Coordinate and document changes and updates to the PCH Assessment on the medical evaluation and support plan.
  • Be knowledgeable of resident care outcome trends and issues on the unit.
  • Implement strategies for reducing medication errors, resident falls, healthcare-acquired infections, pressure ulcers, improving call bell response time, and reducing hospital transfers and readmissions.
  • Audit Medication Administration Records (MARs) at the end of the month.
  • Assist the Administrator in implementing systems to ensure compliance with all applicable local, state, and federal regulations and accreditation standards, including participation in QA/compliance committees, monitoring, auditing, reporting, mock surveys, dashboards, and DHS inspections.
  • Complete weekly unit inspections using the criteria and format required by the Administrator, and submit the report with a corrective action plan weekly.
