About The Position

Brookestone Home Health & Hospice is seeking a Full-Time Licensed Practical Nurse (LPN) to join their Home Health & Hospice team serving Grand Island and surrounding communities. This role involves providing personalized nursing care, working closely with patients, families, physicians, and an interdisciplinary care team to improve quality of life. The position focuses on building meaningful connections and providing compassionate, one-on-one care to help patients maintain comfort, independence, and dignity in their homes.

Requirements

• Current, active Nebraska Licensed Practical Nurse (LPN) license
• Current CPR certification
• Strong clinical assessment and communication skills
• Compassionate, patient-centered approach to care
• Excellent organizational and time-management abilities
• Ability to work independently while collaborating effectively with a team
• Commitment to providing exceptional service and care

Responsibilities

• Deliver skilled nursing care in patients' homes
• Monitor patient conditions and communicate updates to the healthcare team
• Assist with implementing individualized care plans
• Educate patients and families on health conditions and care needs
• Provide comfort-focused care and support to hospice patients and their loved ones
• Promote patient independence, safety, and overall well-being
