About The Position

The LPN provides nursing care to a select group of residents in an assisted living and memory care unit under the supervision of a SALSA. The role provides supportive and restorative care, teaches residents and families, and collaborates in implementing resident services plans. The position is located at Masonicare at Mystic in Mystic, Connecticut, and is for 24 hours per week on the evening shift, including every other weekend.

Requirements

Graduate of an accredited School of Nursing.
Current LPN license.
CPR certification.

Nice To Haves

1–2 years of clinical experience in a hospital or similar health care setting is preferred.

Responsibilities

Collect resident data based on age, needs, and abilities on an ongoing and systematic basis; contribute to resident care and service plans and participate in regular care planning meetings.
Communicate and collaborate effectively with residents, families, colleagues, and other members of the health care team to meet resident needs.
Educate, supervise, and counsel residents and families regarding assisted living procedures and other care needs.
Document accurate, pertinent, and comprehensive information in the client service record in a timely and concise manner.
Conduct supervisory visits of resident care assistants, companions, and homemakers as required.
Participate in educational programs, in-services, meetings, and workshops, and use current literature to enhance professional skills and knowledge.
Under the supervision of a SALSA/RN designee and in collaboration with physicians, pharmacists, and outside agencies, oversee medication management, including ordering, storing, reconciling, and preparing medications for administration and/or CNA prompt.
Perform other duties as assigned.
