About The Position

Grey Stone Health & Rehab is hiring a full-time Licensed Practical Nurse for the evening shift, 2 p.m. to 10 p.m. The LPN uses nursing skills to promote residents’ physical and psychosocial well-being and builds meaningful relationships by learning about residents’ and staff members’ life stories. The role includes medication management, resident assessments, and documenting changes in resident condition. The nurse supports a comforting, engaging atmosphere and works collaboratively with nursing and other facility teams to provide coordinated care. Team members are expected to demonstrate Compassion, Accountability, Relationships, and Excellence (C.A.R.E.) in their interactions with residents, families, customers, and staff.

Requirements

Current and valid Licensed Practical Nurse license in the state of Indiana.
Proficient medication management skills.
Ability to conduct thorough assessments and accurately document changes in resident condition.
Strong passion for geriatric nursing and commitment to senior care excellence.
Excellent communication and interpersonal skills.
Ability to work as part of a team toward excellent resident care.
